Internal policies in IT Organizations, especially those in heavily regulated environments, have become overly complicated through reactionary decision making, band-aids, and "that's the way we've always done it" reasoning. We got here by trying to be responsive to the ever-changing needs of the business and using the tools and approaches we know well, but the result often resembles a Rube Goldberg machine that constantly impedes its own best efforts, and the more we try to fix it the worse it gets.
